Llenado capilar como predictor de respuesta a volumen en sepsis. Artículo de revisión teórica

Resumen

El tiempo de llenado capilar (TLC) permite estimar, de forma simple y no invasiva, el estado de la microcirculación. Es un parámetro de utilidad en la valoración de pacientes con sepsis. Este estudio tiene como objetivo describir la utilidad del llenado capilar como predictor de respuesta a volumen en sepsis. El análisis se realizó en pacientes adultos en el departamento de emergencias, mediante una revisión teórica de la literatura. Como fuentes de información se tomaron publicaciones científicas en bases de datos especializadas: TripDatabase y Medline; con los buscadores Pubmed y Google académico. Se incluyeron artículos científicos de revistas indexadas, publicados desde 2012-2022, en los que se haya determinado la utilidad del llenado capilar como predictor de respuesta a volumen en sepsis, o choque séptico, en adultos. Se realizó un diseño analítico, con riesgo de sesgo bajo, calidad de la evidencia media a alta. Como principales resultados se incluyeron 12 estudios primarios, a partir de los cuales se evidenció que, el tiempo de llenado capilar pudiera ser una opción válida en la valoración inicial de los pacientes con sepsis y choque séptico. Tomando un punto de corte de 3 segundos, este parámetro permite establecer, con bastante certeza, la respuesta del paciente al tratamiento con fluidos, su pronóstico y probabilidad de fallecer, en el contexto del departamento de emergencias. La determinación del TLC puede ser una opción válida en la predicción del pronóstico la sepsis en adultos, en el departamento de emergencias; especialmente en contextos de bajos recursos. Junto a la determinación de los niveles de ácido láctico, su utilidad parece incrementarse, sin embargo, es preciso contar con más investigaciones al respecto.
