Absceso hepático como precursor del cáncer colorrectal

Resumen

Los abscesos hepáticos (AH) piógenos criptogénicos han sido denominados precursores del cáncer colorrectal (CCR). La importancia de la relación entre estas patologías reside en el diagnóstico y tratamiento temprano del cáncer, por lo que la investigación de las causas subyacentes al desarrollo de los abscesos hepáticos criptogénicos es requerido. El objetivo de esta revisión fue exponer los estudios que muestren la prevalencia de los AH como signo precedente del CCR, a partir de lo cual, se identificó un rango del 1,9 al 13,9%, según estudios de poblaciones asiáticas. La ubicación, número, sintomatología de los AH no mostraron relación específica con la patología intestinal, mientras que la diabetes mellitus y la presencia de klebsiella pneumoniae fueron los factores incidentes en la mayoría de los casos de CCR. Uno de los mecanismos identificados fue la translocación bacteriana desde la pared intestinal alterada, la patogenia de los microorganismos implicados y estados inmunocomprometidos. En conjunto, estos factores determinan el desarrollo de abscesos hepáticos como precursores de procesos neoplásicos colónicos.
