Proposal for classification of acute pancreatitis based on tissue microcirculation damage

Abstract

Acute pancreatitis continues to be an important disease in terms of incidence, mortality and cost. Classifications and Severity Scores have not taken into account the pancreas microcirculation. The development of new techniques assessing metabolic and hemodynamic status of the pancreatic microcirculation make perfusion study possible. Objective. To propose a new classification of acute pancreatitis based on complementary studies that show the impairment of pancreatic microcirculation. Methodology. A Google Scholar, PubMed, and Cochrane Library Spanish and English review of the pathophysiology, molecular biology, and changes in pancreatic perfusion evidenced in high-tech complementary studies was performed. Results. A new classification of acute pancreatitis was developed based on the changes occurring to the microcirculation and perfusion of the pancreas, which could be a useful indicator of severity. Conclusion This classification is very realistic as it is based on the involvement of the organ's microcirculation according to the results of complementary studies, it also offers a personalized guide to the fluids infusion rate and the drugs required to improve perfusion. Further investigations are needed to complement this research.
