Use of methylprednisolone in patients with spinal cord trauma, current situation

Traumatic spinal cord injury (TSCI) refers to the injury caused by external physical impacts, which can generate contusion, laceration, massive compression and solid spinal cord injury. The incidence rates of TSCI vary from 13 to 220 per million population. The pathophysiological events that occur after the injury are classified into two phases, one immediately after the trauma (primary injury) and another that occurs days to months later (secondary injury). For the treatment of TSCI, several neuroprotective strategies are used, such as the use of methylprednisolone, control of mean arterial pressure, and surgical decompression, but the safety and effectiveness of new drugs and substances in the acute treatment of TSCI are currently being evaluated. patients with TSCI. More scientific publications are needed to provide more evidence for the use of these new strategies. The literature is analyzed in search of the pathophysiological foundations of the use of corticosteroids in Spinal Cord Trauma (SCI). The most relevant articles on the use of methylprednisolone in MRT are reviewed, and they are critically analyzed, both in their design and in the way of presenting their results. It follows that the use of high-dose methylprednisolone in Acute Spinal Cord Trauma should be used with caution, and being alert to possible complications.
