Capillary refill as a predictor of response to volume in sepsis. Theoretical review article

The capillary refill time (TLC) allows to estimate, in a simple and non-invasive way, the state of the microcirculation. It is a useful parameter in the assessment of patients with sepsis. This study aims to describe the usefulness of capillary refill as a predictor of response to volume in sepsis. The analysis was performed in adult patients in the emergency department, through a theoretical review of the literature. As sources of information, scientific publications in specialized databases were taken: TripDatabase and Medline; with the Pubmed and Google academic search engines. Scientific articles from indexed journals, published from 2012-2022, in which the usefulness of capillary refilling as a predictor of response to volume in sepsis, or septic shock, in adults has been determined, were included. An analytical design was carried out, with low risk of bias, medium to high quality of evidence. As main results, 12 primary studies were included, from which it was evidenced that capillary refill time could be a valid option in the initial assessment of patients with sepsis and septic shock. Taking a cut-off point of 3 seconds, this parameter makes it possible to establish, with considerable certainty, the patient's response to treatment with fluids, their prognosis and probability of death, in the context of the emergency department. The determination of the TLC can be a valid option in the prediction of the prognosis of sepsis in adults, in the emergency department; especially in low-resource settings. Together with the determination of lactic acid levels, its usefulness seems to increase, however, more research is needed in this regard.
